SFH FMR Bias Reporting Tool

The following form is to report any incidents of bias. This form can be used by staff, residents, or faculty to report these incidents.

What we do with bias incident reports:

  • Incidents reported will be sent directly to the Program Director. If you wish to send the form to the administrative staff of SFH FMR as an alternative please use the secondary form below.

  • Refer those who report incidents to appropriate resources that can effectively respond and support individuals.

  • Catalog all reported incidents and track for trends. Trends or incidents, if allowable, will be shared with the Inter-sectional Justice Committee to allow for programmatic improvement. If applicable, feedback to individuals will be provided in a confidential setting.

  • Assess avenues for minimizing or eliminating future incidents of bias while working with Swedish DEI departments on strategies for systematic change.

Submission of a Bias Incident Report does not constitute a formal complaint and acts of overt bigotry or harm should be report with formal avenues of communication. Formal avenues should be utilized with Swedish HR which can be found on the Swedish intranet at http://swedishonline.swedish.org/Departments/EH/HR/Pages/default.aspx and the Program Director at ben.davis@swedish.org
